Thank you for your interest in Child and Family Services' Home Care Program.

Our goal is to help seniors and individuals with disabilities to live at home with dignity for as long as it is safe and they choose to do so.

Companions, homemakers, personal care service providers, licensed nurse’s aides (LNA), and nurses provide individuals with assistance in household tasks, errands, personal hygiene, medication pre-fills and reminders, and other activities of daily living.

Child and Family Services' Home Care staff are dedicated, compassionate, mature, and responsible.  Staff work under the direction of a registered nurse and follow individualized plans of care.

Services are paid for privately, through the Medicaid Choices For Independence Program, Title XX, and Title IIIB.

Funding

The CFS Home Care program is funded in part by a Manchester City Improvement Project grant, NH Department of Health and Human Services, the Mary Gale Foundation and the Pearl Manor Foundation.

 

 

Fees 

Services are paid for privately, through the Medicaid Choices For Independence Program, Title XX, and Title IIIB.   A sliding fee scale is available.
